How We Work
At Chase Home, we are committed to providing specialized care for youth facing extraordinarily complex circumstances.
Our programs are accessible both in the community and at The Chase Home where some youth live while we work with them and their families. Our programs are intentionally designed to help youth (and their families) address the causes behind issues that threaten their happiness and well-being.

Our dedicated team is experienced in supporting young individuals dealing with a diverse range of challenging situations. Many of the youth we assist face substance addiction, histories of parental abuse, instances of sexual trafficking, and experiences of abandonment, among other distressing scenarios.
The youth we serve are confronting some of the most pressing societal challenges, and our mission is to offer them the highest level of professional care and support.
The Types of Services We Provide
Clinical Support(s)
These services encompass a wide spectrum of therapeutic modalities, tailored to address the unique needs and challenges faced by each youth. From cognitive-behavioral strategies to expressive arts therapy, we employ proven methods within a caring and nurturing environment to foster emotional, social, and cognitive development.
Life Skill Training(s)
In addition to our clinical focus, we emphasize the cultivation of practical life skills that are crucial for a successful transition into adulthood. This emphasis includes but is not limited to: financial literacy, vocational training, academic support, and independent living skills. We believe that by equipping our residents with these essential tools, we help them take crucial first steps that will lead to fulfilling and self-sufficient lives.
Permanency and
Family-Involvement
Family involvement is at the heart of our approach. We recognize the vital role that families play in a young person’s journey towards stability and growth. Our programs actively encourage and facilitate open communication, providing families with the necessary resources and support to foster a positive and nurturing home environment.
Our Programs
At The Chase Home, we understand that every young person’s journey is unique. We offer individualized care plans that evolve alongside our participants – empowering them with the skills, resilience, and confidence they need to forge a path towards a brighter, more promising future.
Our programs provide a safe and nurturing environment where youth can embark on a path towards healing and resilience.

Residential Treatment Services
On-Site
Our Residential Treatment Services (RTS) program offers intensive therapeutic support and care for youth (ages 11-18) who have been referred to us through the Department of Children, Youth, and Families (DCYF) or those who would otherwise be homeless or unable to have their needs safely met at home.
We understand that foster youth, like their peers, need the guidance of caring adults to develop the skills essential for success. Each young person works with us to set their own goals and outline the steps needed to achieve them. This journey often includes individual therapy to address past traumas and group sessions focused on building coping skills and resolving conflicts.
While living at The Chase Home, these young individuals:
- Attend local schools
- Engage in extracurricular activities
- Contribute to household chores
- Hold jobs in the community, if they are of age

Independent Living Program (ILP)
On-Site
Our Independent Living Program (ILP) supports young adults aged 18-22, who age out of residential care and have no permanent family they are able to live with. Some have been referred by the Department of Children, Youth, and Families (DCYF), as they take their first steps toward independent living, and some directly transition from our Residential Treatment Service Program.
Each year, The Chase Home assists 5-6 young adults in finding their first apartment, securing a job, obtaining personal documents, signing the lease, covering all necessary deposits, and purchasing essential furniture.
Young adults in our Independent Living Program:
- Attend school, participate in educational programs, and/or have a job.
- Manage their own grocery shopping, cooking, and cleaning.
- Meet with Chase Home staff weekly.
- Actively participate in developing their personalized, youth-guided treatment plans.

Home-Based Program
Community-based
Our Home-Based Therapy (HBT) and Individualized Service Option (ISO) programs provide flexible customized clinical and therapeutic services to at-risk youth who are experiencing serious difficulties around several areas in their lives. These services are available in the community and inside the homes where youth live with their families. These programs are designed to either prevent placement in our Residential Treatment Services (RTS) program or help youth transition back from RTS to a community and family-based setting. Our Child Health Support (CHS) program aims to provide continued support to children and their caregivers who are navigating the path to reunification.

Seacoast Community Diversion Program (SCDP)
Community-based
The Seacoast Community Diversion Program (SCDP) is an early intervention program that uses restorative justice based principals to divert youth between 13 and 18 who have committed a chargeable offense from the traditional juvenile justice system (and probation).
